Forms
The Birmingham Bar Association intends to maintain online a collection
of forms prepared by our members in connection with legal matters
they have previously handled.
These forms are in Adobe Acrobat (PDF) format, and it will be necessary
for you to download and install the Acrobat Reader in order to access
them. In some instances, the forms are of the pre-printed variety,
which have likewise been placed in Adobe Acrobat (PDF) format. However,
with respect to these printed forms, there are a couple of add-ons
which will permit you or your staff to complete the forms from within
your web browser and print the completed form for filing or mailing. The
buttons below will connect you to the appropriate files for downloading.
You need to download the reader and plug-in only once.
